Job Descriptions - Night and Day Shifts
Chateaux Serenity Assisted Living

Caregiver General Job Description

Including but not limited to: (see attached day or night job description)

Resident Care – Bathing, dressing, personal care, oral care, ambulation assistance, incontinence care, feeding assistance, monitor vital signs

Meals – prepare, serve, clean, snacks, grocery list, encourage fluid intake

Housekeeping – follow cleaning schedule

Home maintenance – safety, any unsafe conditions will be reported to management immediately

Management – complete all paperwork in timely fashion including: charting, medication forms, time sheets, inventory forms, others.

Follow policy and procedures

Participate in all fire drills and evacuation drills

Participate in activities: assisting residents, notifying management of supplies needed

Attend mandatory in services/case management meetings/other meetings as required/requested

Maintain in service hours as required

As job description changes for any reason, the employee will be notified by writing.

 

Caregiver Day Shift Job Description

  • Shift Change Report: beginning of shift
    • Count controlled medications
    • Take resident vital signs
    • Check each resident
    • Get medication key from outgoing caregiver
  • Ongoing during the day:
    • Care for residents
    • Assist residents with medications and treatments as needed
    • Check vital signs as needed; contact DON with any abnormality
    • Do activities with residents each day
    • Warm and serve cooked food
      • Feed residents five times a day: 3 meals, 2 snacks
    • Dust and mop resident rooms, including window sills
    • Take out each room’s trash, including plastic bags; replace plastic bags
    • Check for any obstructions on any floor
    • Keep the refrigerator clean and organized
      • Monday, Wednesday, Friday: check for expired food, discard as needed
    • Change linen as needed, AND after each shower – minimum weekly
    • Put linen in laundry room, separated, for night shift
    • Put away groceries
    • Stock laundry room and all bathrooms
    • Clean front door as needed
    • Perform hourly rounds for residents
    • Contact DON with any change of condition of residents
    • By 6:00 to 6:15, open common area blinds, put relaxing music on main tv

End of shift:

  • Empty trash again before shift change, replace plastic bags; take to big trash can, Do NOT leave for next shift
  • Perform shift change report with incoming caregiver, hand over medication key

 

Caregiver Night Shift Job Description

  • Shift Change Report:
    • Count controlled medications
    • Take resident vital signs
    • Check each resident
    • Get medication key from outgoing caregiver

8:00 to 4:00 am

  • Assist residents with medications and treatments as needed
  • Check vital signs as needed; contact DON if any abnormality
  • Laundry, washed and folded – whites and colors separated
  • Mop floors, all common areas; use cleaning products provided (bleach, Fabulosa)
  • All trash emptied: kitchen, bathrooms, each resident room, including the plastic bags; Trash is to be taken to big trash can out back Tuesday and Thursday nights:  big trash can taken to street for pickup
  • Bathrooms cleaned: floors, toilets, sinks, showers, countertops
  • Dust living spaces, including all tables, furniture, shelves
  • Clean and disinfect the kitchen bar and all countertops
  • Wipe down all door jambs and kitchen cabinets
  • Refill house supplies: med cart, tissue in rooms, folded paper towels in baths
  • Sunday: clean out refrigerator, discard any expired food
  • Perform hourly rounds for residents
  • Care for residents as needed
  • Contact Director of Nursing with any change of condition of residents

End of shift:

  • Check for trash, replace plastic bags
  • Take trash to big trash can as needed, do NOT leave for next caregiver
  • Perform shift change with incoming caregiver
